Houston, TX (Field)
Registered Nurse (RN)
Provide skilled nursing visits and supervise care plans for adult and pediatric members. Conduct OASIS assessments, administer medications, manage wound care, and coordinate with physicians. PDN nurses manage high-acuity cases including tracheostomy and ventilator-dependent members.
Requirements
- Current Texas RN license
- Minimum 1 year clinical experience (home health preferred)
- BLS certification
- Valid Texas driver's license and reliable transportation
- Willingness to work high-acuity PDN cases (trach/vent experience preferred)

Full-time / PRN

Licensed Vocational Nurse (LVN)
Provide skilled nursing care under RN supervision for adult and pediatric members. Medication administration, vital signs, wound care, and member education. LVNs with PDN experience are especially valued for our high-acuity caseload.
Requirements
- Current Texas LVN license
- Clinical experience in home health or acute care
- BLS certification
- Valid Texas driver's license and reliable transportation
- Dependable and detail-oriented with strong documentation skills

Houston, TX (Field)
Personal Care Attendant (PCA / HHA)
Our PAS attendants are the backbone of daily member support. You'll help with bathing, grooming, meal preparation, mobility, light housekeeping, and medication reminders — all under RN supervision through the Medicaid PAS program. We train. If you are dependable, kind, and show up on time, we will teach you the rest. EVV clock-in and clock-out every shift is mandatory and is how you get paid.
Requirements
- 18 years or older and legally authorized to work in the U.S.
- Clean criminal background (no barred offenses under Texas Health & Safety Code §250) — we run the check
- Current TB screening — we'll arrange one if you need it
- Smartphone capable of running the Texas EVV app (iPhone or Android, not a flip phone)
- Reliable transportation to member homes across Greater Houston
- Caring, punctual, willing to learn. HHA, CNA, or prior caregiving experience is a plus — not required.
